New Patient Information
Referral vs Direct Access
Some insurance companies require a physician referral prior to starting physical therapy. If this is the case, you can request physical therapy from your primary care physician. They will fax us or provide you with a script, which you can bring at your first appointment. However, New Hampshire is a direct access state meaning that a physician referral is not always required. If you're not sure if you will require a referral, please reach out as we will be happy to assist you.

What to Expect
Please allow 1 hour for your first appointment. During your first appointment you can expect your therapist to perform an initial evaluation and administer therapeutic treatment. In addition, you will work with your therapist to develop a plan of care and a home exercises that will further assist in your recovery. See below for additional information.

Patient Paperwork
Contact us by phone or email to schedule your initial evaluation. You will receive a follow up email with new patient paperwork to complete prior to your first visit. If you are unable to complete your paperwork at home, please plan on arriving 15 minutes early to your first visit to allow time to complete in the clinic.

Insurance
Make sure to call your insurance company prior to your first appointment so your benefits can be verified. A physician referral is not always necessary for insurance coverage.
